Questions for Insurance

Prior to your initial appointment, call the number on the back of your insurance card to verify coverage. Ask them the following questions:

 

"Do you reimburse for the following CPT codes: 97802 (initial nutrition assessment) and 97803 (follow-up nutrition assessment)? How much do you cover?  

"Do you reimburse for ICD-10 code Z71.3 (dietary counseling and surveillance)?"

If a medical diagnosis is required, have your physician fill out this referral form and bring the completed document to your appointment.
